The world has witnessed a remarkable achievement in the realm of global health: the Big Catch-Up initiative has successfully delivered over 100 million vaccines to children in 36 countries. This is a significant milestone, but it also serves as a stark reminder of the ongoing challenges in global health and the need for continued vigilance. Personally, I think this achievement is a testament to the power of international collaboration and the importance of addressing health disparities, but it also highlights the complex issues that persist in global health. What makes this particularly fascinating is the contrast between the success of the Big Catch-Up and the ongoing struggles with routine immunisation. While the initiative has made remarkable progress, it also underscores the need for sustained efforts to ensure that every child has access to essential vaccines. From my perspective, the Big Catch-Up is a shining example of what can be achieved when governments, organisations, and communities come together to tackle a critical global issue. However, it also raises important questions about the underlying causes of vaccine hesitancy and the need for more effective strategies to address it. One thing that immediately stands out is the disparity between the success of the Big Catch-Up and the persistent challenges with routine immunisation. While the initiative has reached an estimated 18.3 million children, including 12.3 million zero-dose children, the fact that 14.3 million infants globally failed to receive a single vaccine through routine immunisation is deeply concerning. What many people don't realise is that the Big Catch-Up is not a silver bullet. While it has made significant progress, it is essential to understand that it is just one piece of the puzzle. The initiative has provided life-saving vaccines to children who would otherwise have been missed, but it has not solved the underlying issues that prevent routine immunisation. If you take a step back and think about it, the Big Catch-Up is a response to the COVID-19 pandemic, which has disrupted routine immunisation services and led to a decline in vaccine coverage. However, the pandemic is not the only factor contributing to the challenges with routine immunisation. Other factors, such as vaccine hesitancy, logistical challenges, and resource constraints, also play a significant role. This raises a deeper question: how can we ensure that every child has access to essential vaccines, not just during a crisis but on an ongoing basis? A detail that I find especially interesting is the contrast between the success of the Big Catch-Up and the ongoing struggles with measles outbreaks. While the initiative has provided measles vaccines to millions of children, measles outbreaks are rising in every region, with around 11 million cases in 2024. What this really suggests is that while the Big Catch-Up has made significant progress, it is not a panacea. We need to address the underlying causes of vaccine hesitancy and the challenges with routine immunisation to ensure that every child has access to essential vaccines. In my opinion, the Big Catch-Up is a crucial step forward, but it is just one piece of the puzzle. We need to continue to invest in routine immunisation programmes, address vaccine hesitancy, and work towards ensuring that every child has access to essential vaccines.
